How Does Mentoring Work?

Our Lifestyle Support Program offers one-on-one mentoring that helps people build everyday skills, reconnect with their community, and work towards a life they choose. We match each participant with a mentor based on their goals and needs, ensuring a person-centred, supportive relationship.

Our mentors are trained to work with people experiencing mental health challenges, disability, dementia, or behaviours of concern. Support is flexible and may include building social skills, returning home after hospital, developing daily routines, or increasing independence.

Our Process

We follow a clear, person-centred process-mentoring to ensure support is effective, ethical, and collaborative.

1. Start with a Referral & Initial Conversation

Every journey begins with a referral and a meaningful conversation to understand the individual’s needs, goals, and personal context.

2. Matching with a Mentor

We carefully match each participant with a mentor based on their goals, location, and availability. A mentor profile is shared in advance, so participants know who they’ll be working with.

3. Exploring Goals Together

In the early sessions, the mentor works with the participant to explore what matters most to them—building trust and identifying initial goals.

4. Optional Goal-Setting Workbook

Participants can choose to use our flexible Goal Setting Workbook, designed to help break down aspirations into clear, achievable steps—guided by their mentor.

5. Flexible, Participant-Led Sessions

Mentoring is dynamic and responsive. Sessions are guided by the participant’s needs on the day, with mentors bringing experience and adaptability to support growth, whatever the focus.

6. Support from the LSP Coordinator

Our dedicated Lifestyle Support Program Coordinator is always on hand to assist with scheduling, changes, or general support—ensuring the experience stays smooth and responsive.

Mentors can support with social skills, routines, community access, work readiness, confidence building, and goal-setting—based on what’s important to each participant.

We match mentors based on the participant’s goals, preferences, and location to ensure a person-centred fit that fosters trust and connection.

Mentoring is typically funded under NDIS Capacity Building or Core supports, but we’re happy to discuss individual circumstances and explore options with you.
